By checking this box, the guardian(s) of the above named player hereby give permission for her to participate in North Mason Fastpitch (NMF) formally known as NMGSA for the 2024 season. I/we understand that injuries may result regardless of precautions or protective equipment, and do hereby waive, release, and agree to hold harmless the NMF, including the organizers, coaches, participants or any other persons affiliated with this organization, for any injury to my child while participating, attending, or going to or returning from any event.

By checking this box, the guardian(s) of the above named player hereby give permission for her photograph to be taken in the course of any NMF event. I/we understand that it can be used for program promotion, recruitment and/or advertisement, in any media form.

A refund minus insurance fees will be available until 03/05/2024. A refund will not be available after 03/20/2024. The League will not be Providing pants this year this will be the players responsibility. The color of the pants will be decided by the team once teams are set. The league will have a limited supply of discounted pants that will be available for purchase.
